It's for clearance....there's not enough room on the inboard side of the framerail for a regular nut to fit. I don't remember exactly what it is, but there is something right there in the way. I've never seen that crazy looking fastener before. I have aftermarket tow hook brackets and the kit just used carriage bolts through the frame rail.
