As I get older I forget things much more often. It's called a chime module and is located next to the fuse panel. There are two varieties, the blue case and the black case. The blue case has the headlamp warning; the black case does not. Plentiful at the junkyards or I'll send the part number if you want to buy one.

If I turn the truck off with the headlights on and THEN push the switch in they will stay on for about 2 or 3 min and automatically shut off. but IIRC i have never heard a buzzer for my headlights, but i have heard one for my seatbelt.

Different module - the headlight delay is controlled by the Head Lamp Sentinel module. Has nothing to do with the chime module.
