What is this thing and do I need it?

That is the spare tire holder.  It lowers and raises the spare tire under the bed.

 

Yours looks like it has seen better days.

Tim is correct, that is your spare tire winch. If you're not going to have a spare under the bed, you don't need it. Don't throw it away though, they are restorable and someone may want it.

Any good write-up on the restoration you happen know of, especially ones with good pictures?
