A/C vent hoses routing

Is there a good diagram available for the hoses to the vents? Not sure about the vents at face level but the one under the column is definitely missing. Where does it go under the dash and how difficult is it to fix (how much dash removal)? I'm sure that I have a vacuum leak some where due to the air only coming from the defrost vents.

Thank you. Along with your pic I did find some good info on where to start on the vacuum leak issue, but I'm trying to find info on the vents themselves. Where do the a/c vents attach behind the dash?

I'll bet you fix the defrost issue and your other issues will magically disappear.

I found the leak by the firewall and the air blows from the vents now. Thanks for the pics and tips.
