Sachs Clutch blow up

Hind sight is 20/20. We went cheap and got an O'Reilly's Sachs replacement clutch for Dad's 94' F250. It lasted about 4-5 months. It had a lifetime warranty but I've already went and bought a better Napa product. Luckily the flywheel wasn't eaten up at all, still smooth and didn't even have burn marks on it...I'll scuff it up a little and go in with the new clutch set.

:dunno: I work at an O'Reilly's and have sold tons of Sachs clutches, the only reason I've seen them come back is due to missing parts. That said we just switched from Dynapak to Sachs about 8 months ago, I've got a Dynapak disk and pressure plate with a Sachs slave in my MJ.

It looks to me like a clutch drop was performed. Not trying to blame anything but with that kind of damage if it was not deliberetly done have you checked the pressure plate? Maybe you have a mis-match setup? That is a wasted clutch friction disk for sure but with that kind of carnage something's wrong. The flywheel should be pretty tore up too. Pics?

No, we didn't abuse it, just normal driving and no towing or heavy loads. Most of the time it doesn't even need much clutch to take off due to the 4.11 axle gears and the granny gear in the transmission. I think the pressure plate failed and just couldn't hold enough pressure and it just burned to a crisp.
