power steering reservoir cap flew off?

Any reason for this besides just not being tightened properly?

XJs and MJs are known to do this. There's a lot of pressure in a PS system and it can blow the cap off. It's happened to a few people I know. I keep a spare one in my toolbox now.

Any fluid being forced out of the cap vent hole? It might be clogged and can't vent...........

I had this issue before I went to my AGR setup. Those caps are not the cheapest either. After the 4th one I ended up putting a little chain on it so I didn't loose the cap any longer. I just used a self tapping screw directly into the top of the cap and than hooked the other end of the chain around the line.
