need help picking valve cover

i need to get a cover to prevent oil blow back

 

what year, make and model should i get this from?

and is there anything special that has 2 b done when i install, besides gasket?

i know its in like 100 threads, but i need to know the right one for sure cuz I'm going this morning :bowdown:

and what hoses do i need if any?
